Abstract
Objective To observe the clinical effect of dexmedetomidine on the treatment of children with tonsillectomy and adenoidectomy by low temperature plasma knife.Methods Totally 60 children with ASA Ⅰ ~ Ⅱ were conducted with tonsillectomy and adenoidectomy,and were randomly divided into Dex group and group C,30 cases in each group.Both groups were treated with general anesthesia and endotracheal intubation intravenous inhalation.Midazolam,fentanyl,propofol and atracunum besylate were applied sequentially,and the fentanyl,propofol,sevoflurane were used as maintenance anesthesia.In Dex group,loading dose of dexmedetomidine was used 10 min before induction,and then changed to the maintenance dose.Group C was pumped with normal quantity physiological saline.The change of vital signs and extubation time were compared between two groups,and the mean blood pressure (MBP),heart rate (HR) and respiratory rate (RR) at the time points of entering operating room (T0),the beginning of the operation (T1),1 min after extubation (T2),5 min after extubation (T3),15 min after extubation (T4) and dysphoria score at each time point as well as Ramsay score were recorded.Results Children in both groups obtained good anesthesia effect during operation,and there were no significant differences in intra-operative blood pressure and heart rate.After the surgery,there was no significant difference of spontaneous breathing recovery time between two groups.Compared with group C,dysphoria score after extubation in Dex group was significantly lower (P < 0.05),and Ramsay score was significantly higher (P < 0.05).Conclusion Dexmedetomidine can obviously reduce the occurrence of postoperative dysphoria and effectively stable hemodynamics in children with tonsillectomy and adenoidectomy by low temperature plasma knife.关键词
