老年肾功能不全者补钙致高钙血症误漏诊六例分析

Abstract
Objective To explore the clinical features of hypercalcaemia caused by calcium supplements in elderly patients with renal insufficiency and reasons for misdiagnosis.Methods The clinical characteristics of 6 patients with hypercalcemia were analyzed and the related literature was reviewed.Results The course of disease was 1-4 weeks,including 3 cases misdiagnosed as gastroenteritis,1 as cerebral infarction,1 as Alzheimer's disease,and 1 case of missed diagnosis.Three patients reported fatigue,dry mouth,nausea and vomiting;2 cases showed hypercalcemia crisis,such as being apathetic,loss of appetite,nausea,vomiting and disturbance of consciousness;1 case had no obvious clinical symptoms.Blood biochemical tests revealed renal insufficiency and elevated serum calcium.Based on further questions about the patient's history,all 6 patients had a long history of oral administration of various calcium drugs,and were therefore diagnosed as having hypercalcemia.They were discharged home with a better condition after symptomatic treatment.At 6-month follow-up,no abnormal serum calcium levels were found.Conclusion Elderly patients with renal insufficiency should pay attention to standard use of calcium and active vitamin D as well as regular monitoring of serum calcium,which is the key to avoid misdiagnosis of hypercalcemia.关键词
