Abstract
Objective:To analyze therapeutic effect of hysteroscopic surgery for different types of Cesarean scar pregnancy(CSP).Methods:Seventy patients diagnosed with CSP were selected in Shengjing Hospital affiliated to China Medical University from January 2013 to June 2016.All the patients were divided into three types:Type Ⅰ (n=32),Type Ⅱ (n=26),Type Ⅲ (n=12) according to the CSP classification criteria proposed by the Chinese Medical Association of Family Planning.All the patients were treated by hysteroscopic resection of cesarean scar pregnancy.The operation condition,hysteroscopy success rate,postoperative recovery and other pregnancy outcome were compared among the three groups.Results:The operation time in Type Ⅲ was significantly longer than that of Type Ⅰ and Ⅱ (P＜0.05).The bleeding volume in Type Ⅱ and Ⅲ was significantly more than Type Ⅰ (P＜0.05).There was no significant difference in the duration of vaginal bleeding among the three groups (P＞0.05).No complications occurred in all three groups.The single hysteroscopic operation success rate in Type Ⅰ was 96.9％,and Type Ⅱ and Ⅲ was 84.6％ and 50％ respectively.The success rate in Type Ⅰ and Ⅱ was significantly higher than Type Ⅲ.There was no significant difference in the menstrual recovery time and the time when HCG fell to the normal range Mter outpatient visits among the 3 groups(P＞0.05).The interval of ultrasound images returned to normal condition in Type Ⅲ [(42.9±5.2) days] was significantly longer than that of Type Ⅰ and Ⅱ [(18.9±4.2) and(20.6±3.6) days,respectively](P＜0.05).A total of 12 patients were followed up after pregnancy and no CSP reoccurred.Conclusions:All kinds of Cesarean scar pregnancy can be treated by hysteroscopic surgery,especially for Type Ⅰ and Ⅱ.关键词
