Abstract
Objective To explore the effect of continuous venovenous hemofiltration (CVVH) combined with enteral nutrition intervention in treatment of elderly refractory heart failure.Methods A total of 120 elderly refractory heart failure patients admitted in Zhongxiang hospital of Wuhan University People's Hospital from Feb 2014 to Aug 2016 were enrolled,all patients were divided by random number table into four groups,with 30 cases in each group.Control group received conventional and symptomatic treatment.Nutrition group received additional enteral nutrition intervention.CVVH group received additional continuous venovenous hemofiltration therapy.Combined treatment group received additional continuous venovenous hemofiltration (CVVH) combined with enteral nutrition intervention therapy.Before and after treatment,respiratory rate (RR),heart rate (HR),mean arterial pressure (MAP),natriuretic peptide (NT proBNP),Inflammatory indicator [interleukin 6 (IL-6) and stumor necrosis factor α (TNF-α)],nutrition indicator[serum total protein (TP),albumin (ALB),hemoglobin (HB)] and cardiac function index [mitral flow velocity (SV),cardiac (CO),left ventricular ejection fraction (LVEF)] changes were compared between the four groups.Results Before treatment,four groups' NT proBNP,IL-6,TNF-α,TP,ALB,HB,SV,CO,LVEF showed no significant difference(P >0.05).After treatment,the inflammatory indicator of control group showed no statistically significant (P > 0.05).The index of the three groups were improved (P < 0.05),and the combined treatment group were improved significantly better than the other groups (P < 0.05).Conclusion It demonstrated that the continuous venovenous hemofiltration combined with enteral nutrition intervention revealed good therapeutic effect,it could effectively reduce inflammation,correct nutritional status and improve heart function.关键词
