早期肠内营养对重型颅脑损伤机械通气患者终点事件的影响
The influence of early enteral nutrition on the endpoints of severe brain injury patients with mechanical ventilation therapy in ICU

Abstract
Objective To observe the influence of early enteral nutrition on the endpoints of severe brain injury patients with mechanical ventilation therapy in ICU.Methods Sixty cases of severe brain injury patients with mechanical ventilation therapy in the ICU of our hospital were chosen as study objects,they were randomly divided into experimental group and control group.The experimental group conducted enteral nutrition support in 48h of mechanical ventilation;while the control group conducted parenteral nutrition support until after 7 days of mechanical ventilation.The mechanical ventilation time,7 days nutritional costs,1 week weaning success rate,mortality in 28 days,complication and nutritional indicators of the two groups were compared.Results There was no significant difference in the level of hemoglobin and serum albumin between the 2 groups after 7 days of treatment (t =0.662,t =0.528,P ＞ 0.05);the mechanical ventilation time,7 days nutritional costs of the experimental group were obviously lower than the control group(t =2.153,t =15.520,P ＜ 0.05);and the incidence of gastric reflux of the experimental group were markedly higher(x2 =12.857,P ＜ 0.05);the incidence of VAP and other gastrointestinal complications in the experimental group were significantly lower than that of the control group (x2 =4.091,x2 =4.472,P ＜ 0.05);the 1 week weaning success rate and mortality in 28 days of the two groups had no statistical difference (x2 =1.667,x2 =0.295,P ＞ 0.05).Conclusion Early enteral nutrition can shorten the mechanical time,reduce the cost of nutrition and the incidence of VAP and gastrointestinal complications.The enteral nutrition support should be conducted as soon as possible for the severe brain injury patients with mechanical ventilation therapy in ICU if the patients have normal gastrointestinal condition.关键词
