经皮后路椎弓根钉道强化合并伤椎椎体成形术治疗单节段胸腰椎骨质疏松重度压缩性骨折43例临床研究
Augmented pedicle screws combined with percutaneous vertebroplasty in treatment senile single of level severe osteoporotic vertebral compression fractures -Clinical observation of 43 cases

Abstract
Objective :To explore the curative methods and effect of augmented pedicle screws combined with percutaneous vertebroplasty in treatment of senile single level severe osteoporotic vertebral compression fractures . Methods :From 43 single level thoracolumbar severe osteoporotic fracture patients were treated with percutaneous short segment pedicle screw fixation with bone cement augmentation .The operative time and intraoperative blood loss were recorded .The height ratio of the anterior vertebral column ,the Cobb angle of the local kyphosis and the Visual analogue sacle (VAS) were compared with preoperative and postoperative .Results :The operation time of all patients was 50 ～ 130min ,an average of 72min ,and the amount of bleeding during the operation was 30 ～ 80ml .All patients were followed up effectively ,and the average follow-up time was 15 .4 months (12 ～ 26 months) .The height ratio of anterior vertebral was recovered from the preoperative (41 .61 ± 37)％ to the postoperative (88 .91 ± 3 .95) ％ and the last follow-up (86 .69 ± 3 .39)％ .After local convex Cobb Angle by preoperative 3(1 .6 ± 6 .11)° for correcting to postoperative (11 .91 ± 2 .66)° and at the time of the last follow-up (13 .04 ± 2 .40)° .The VAS score was improved by preoperative (6 .57 ± 0 .95) to postoperative (2 .91 ± 0 .79) and the final follow-up (1 .12 ± 0 . 67) .Postoperative and preoperative comparison were significantly improved .There was no significant difference be-tween the postoperative and the last follow-up .There were no serious complications such as leakage of cement ,fail-ure of internal fixation ,and nerve function injury .Conclusion :Percutaneous short segment pedicle screw fixation with bone cement augmentation in treating single level severe thoracolumbar osteoporotic fracture has advantages of less trauma ,and can effectively restore normal physiological load transmission of spinal ,avoid failure of internal fixa-tion and progress of kyphosis .关键词
