3例局灶性皮质发育不良Ⅰ型诊治思考并文献复习
Diagnosis and treatment of focal cortical dysplasia type Ⅰ:three cases reports and literature review
摘要
Abstract
Objective To analyze the clinical features ,diagnosis and treatment of focal cortical dysplasia ( FCD) typeⅠ,and to improve the understanding and attention of the disease ,and to obtain the correct and reasonable treatment in the early stage.Methods The clinical data of three cases with refractory epilepsy were analyzed retrospectively ,and the clinical features were analyzed in combination with literature review .Results The diagnosis of FCD Ⅰwas confirmed by clinical,imaging,video EEG and postoperative pathological examination .All of the three patients presented with childhood onset,and the clinical manifestations were epileptic seizures .One patient showed positive results of MRI before operation . In the other two cases,there was no obvious abnormality of MRI before operation .One patient showed positive results of MRI before operation .In the other two cases ,there was no obvious abnormality of MRI before operation .Three cases of pa-tients underwent PET-CT examination, Example 1 shows bilateral frontal lobe , left temporal lobe radioactive uptake de-creased;Example 2 shows significantly reduced intake of FDG in the right parietal lobe than the contralateral lobeintake of FDG was significantly reduced;Example 3 shows a high metabolism in the right middle frontal gyrus .All the patients were diagnosed as FCD Ⅰafter operation ,and recovered well without functional damage .Conclusion The onset of FCDⅠwas more prevalent than in the onset of childhood ,FCDⅠcan occur in any cerebral lobe of the brain ,which is often character-ized by medically intractable epilepsy ,head MRI can be negative ,surgical treatment is the main treatment method .关键词
