不同剂量硫酸镁对非体外循环冠脉搭桥患者术后出血的影响
Effects of different doses of magnesium sulfate on post off-pump coronary artery bypass grafting bleeding
摘要
Abstract
Objective:To observe the effects of different doses of magnesium sulfate on postoperative bleeding in patients under-going off-pump coronary artery bypass grafting (OPCABG). Methods:Our study was a clinical randomized controlled trial with 80 ASA III patients,aged 54 ~73,undergoing OPCABG who chose to accept general anesthesia,BMI:20-30. OPCABG was performed with standard technique and device. Patients were intubated tracheal after intravenous sequence induction,connecting the anesthesia machine to intermittent positive pressure ventilation (IPPV). Anesthesia was maintained using the same methods and drugs. Patients were ran-domly divided into four groups:group C (n=20),group M1(n=20),group M2(n=20) and group M3(n=20),saline and 20 mg/kg, 40 mg/kg,60 mg/kg of magnesium sulfate were given from the beginning to the end of surgery. Blood samples were collected 30min be-fore(T0)and 2h after the surgery(T1) to analyze hemoglobin,blood coagulation tests and the serum magnesium level. Postoperative vol-ume of bleeding and transfusion were recorded. Results:Compared with group C,hemoglobin and platelet was decreased significantly in group M3(P＜0. 05),volume of bleeding and transfusion were statistically increased in group M3(P＜0. 05),and the serum magnesi-um concentration in M2group and M3group was significantly increased (P＜0. 05). Compared with T0,hemoglobin decreased in all groups,and the decreased in M3group was more significant at T1(P＜0. 05),the platelet in group M3was statistically decreased at T1 (P＜0.05),serummagnesiumconcentrationingroupCandM1wassignificantlydecreasedatT1(P＜0.05).Conclusion:Pump40 mg/kg of magnesium sulfate during operation does not cause significant bleeding after surgery and can add magnesium ions in OPCABG patients.关键词
