肥胖患者罗库溴铵按去脂肪体重与实际体重间断给药的恢复期药效学比较
Pharmacodynamical comparison of intermittent rocuronium bromide injection base on real body weight and lean body mass pattern in obese patients during long-time surgeries

Abstract
Objective To compare the pharmacodynamics of rocuronium (ROC) bolus injection based on real body weight and lean body mass in obese patients during long -time surgeries.Methods Forty patients (20 normal weight patients, BMI 18～25 kg/m2, Group N; 20 obese patients, BMI 30～35 kg/m2, Group O) scheduled elective surgery (operation time 3-5 h) under general anesthesia were enrolled in the study .The 2 groups were each randomly assigned to 2 subgroups based on difference injection pattern (normal weight +real body weight group , Group NR; normal weight +lead body mass group , Group NL; obese +real body weight group , Group OR; obese +lead body mass group, Group OL).The four-subgroup patients were given an intravenous bolus of double ED 95 of rocuronium according to their real body weight or lean body mass for induction dosage .When T1 recovered to 5%, a dosage of 0.5-fold ED95 was ad-ministered repeatedly for maintaining till 30 min before the end of the operations .The onset time for maximal depression of T1, clinical duration, recovery index and recovery time were determined with TOF -Watch SX accelerometric monitor . And the intubation conditions assessment was also recorded and analyzed .Results There was no significant difference in pharmacodynamic parameters between Group NR and Group NL (P＞0.05).Compared with Group OL, the onset time of Group OR was shortened , while the average administration interval , the time from the end of injection to T 1＝25%, the time from the end of injection to TOF＝0.9, were both obviously prolonged (P＜0.05).In terms of the average adminis-tration interval, the time from the end of injection to T 1＝25%and the time from the end of injection to TOF＝0.9 were significantly proloned in Group OR than Group NR (P＜0.05).Conclusion In long-time surgery, repeatedly adminis-tration of rocuronium in obese patients according can avoid drug accumulation .关键词
