腹腔镜全直肠系膜切除术与腹腔镜辅助经肛门全直肠系膜切除术治疗中低位直肠癌的临床效果及安全性研究
Comparison of Clinical Efficacy and Safety between Laparoscopic Total Mesorectal Excision and Laparoscopic Assisted Transanal Total Mesorectal Excision in Treatment of Patients with Middle and Low Rectal Cancer

Abstract
Objective To compare clinical efficacy and safety between laparoscopic total mesorectal excision and laparoscopic assisted transanal total mesorectal excision in treatment of patients with middle and low rectal cancer. Meth-ods A total of 96 patients with middle and low rectal cancer admitted during January 2014 and June 2016 were divided into study group and control group according to different operative methods ( n=48 for each group). Control group was treated with laparoscopic total mesorectal excision, while study group was treated with laparoscopic assisted transanal total mesorectal excision. Intraoperative and postoperative related indexes and incidence rate of complications were observed and compared in two groups. Results In study group, intraoperative blood loss of was significantly less, while operative time and anal preservation rate were significantly higher than those in control group (P<0. 05); recovery time of gastro-intestinal function, incision healing time, postoperative time of hospitalization and the first time of out-of-bed activity were significantly lower than those in control group (P<0. 05). The total incidence rate of postoperative complications in stud-y group was significantly lower than that in control group (P<0. 05). Conclusion Laparoscopic assisted transanal total mesorectal excision in treatment of patients with middle and low rectal cancer may achieve good clinical efficacy, and it is superior to laparoscopic total mesorectal excision in reducing bleeding volume during operation, promoting recovery of gas-trointestinal function and improve anal preservation rate.关键词
