多层螺旋CT平扫及三期动态增强扫描对直肠癌的诊断价值研究
Value of Multi-slice Spiral CT and Three-phase Dynamic Contrast-enhanced Scanning in Diagnosis of Rectal Cancer
摘要
Abstract
Objective To investigate value of multi-slice spiral CT and three-phase dynamic contrast-enhanced scanning in diagnosis of rectal cancer. Methods Clinical and imaging data of 49 patients with rectal cancer admitted during March 2015 and December 2016 was selected, and results of CT scan and three-phase dynamic contrast-enhanced scan were analyzed. The results of postoperative pathological examination were used as a gold standard to compare detec-tion rate and diagnostic accuracy rate between CT scan and three-phase dynamic contrast-enhanced scan in diagnosis of rectal cancer. Results Detection rate and diagnostic accuracy rate by CT three-phase dynamic contrast-enhanced scan were significantly higher than those by CT scan (P<0. 01). CT scan showed that patients had irregular thickening in lo-cal intestinal wall accompanied by nodular-like changes in some positions; demarcation between diseased intestinal wall and normal intestinal wall was very clear; lesions of 23 patients showed eccentric growth, and the rest showed irregular shape, and no normal intestinal wall morphology was found. CT three-phase dynamic contrast-enhanced scan showed that intestinal canals of patients had different degrees of stenosis and irregular shapes, and cauliflower lesions were more com-mon; mucosal surface was uneven with obvious enhancement in arterial phase of the lesion; lesions in venous phase and delayed phase showed continuously intensified. Conclusion Multi-slice spiral CT three-phase dynamic contrast-en-hanced has a high detection and diagnostic accuracy rates in diagnosis of rectal cancer, and it may clearly show lesion size, internal structure and anatomical relationship with adjacent organs.关键词
