帕瑞昔布钠超前镇痛对老年髋骨骨折患者术后应激反应、镇痛和谵妄的影响
Effects of Parecoxib Sodium on Postoperative Stress Reaction, Analgesia and Delirium in Preemptive Analgesia of Elderly Patients with Hip Fracture
摘要
Abstract
Objective To study effects of Parecoxib Sodium on postoperative stress reaction, analgesia and delir-ium in preemptive analgesia of elderly patients with hip fracture. Methods Clinical data of 78 elderly patients with hip fracture admitted during May 2014 and August 2016 was retrospectively analyzed, and the patients were divided into ob-servation group and control group ( n=39 for each group) according to different analgesic methods. Observation group was treated with Parecoxib Sodium for preemptive analgesia, while control group was treated with traditional analgesic method. In two groups, pain visual analogue scores (VAS) in 6thh, 12thh, 18thh and 24thh after operation were ob-served, and levels of plasma angiotensinⅡ(ATⅡ) and cortisol were detected before and in 12thh, 18thh and 24thh af-ter operation, and then incidence rates of postoperative delirium and adverse reactions were recorded. Results VAS scores in 6thh, 12thh, 18thh and 24thh after operation in observation group were significantly lower than those in control group (P<0. 01). AT II and cortisol levels in in 12thh, 18thh and 24thh after operation in observation group were signif-icantly lower than those in control group ( P<0. 05). Incidence rate of delirium in observation group was significantly lower than that in control group ( P<0. 01). No obvious nausea, vomiting or respiratory depression was found in two groups. Conclusion Parecoxib Sodium in preemptive analgesia of elderly patients with hip fracture may effectively re-duce postoperative stress reaction, improve effect of postoperative analgesia and reduce incidence rate of postoperative de-lirium.关键词
