一例不典型梅尼埃病的临床诊治与探讨
Management of a Case of Atypical Meniere Disease
摘要
Abstract
Objective To have a discussion over clinical features and diagnosis of Meniere's disease through re-view of a case of atypical Meniere's disease. Methods Detailed medical history was collected and thorough physical ex-amination performed. Lab tests included pure tone audiometry, caloric tests, rotatory chair tests vestibular evoked myo-genic potentials (VEMP) and video head impulse test (vHIT) to provide devidence for identification of atypical Me-niere's disease. At the same time, routine conservative treatment of Meniere's disease was provided while closely fol-lowing the disease course. Results The patient was eventually diagnosed with atypical Meniere disease. It was initially cochlear Meniere's disease with recurrent and fluctuating hearing loss but no vertigo, followed by more typical Me-niere's disease presentation exhibiting disequilibrium and dizziness with abnormal caloric test and vestibular evoked myogenic potentials. As a result of conservative Meniere's disease treatment, dizziness and tinnitus significantly im-proved. Conclusion Symptoms in Meniere's disease are variable and atypical Meniere disease does exist. Identifying the disease in its early stages can contribute to accurate differential diagnosis and effective control of the disorder.关键词
