孕期综合干预对妊娠期肥胖孕妇妊娠结局的影响
Effect of comprehensive intervention during pregnancy on pregnancy outcomes of obese pregnant women

Abstract
Objective To explore the effect of comprehensive intervention during pregnancy on pregnancy outcomes of pregnant women with obesity.Methods 180 full-term primiparae with simple obesity in our hospital were randomly divided into intervention group and control group.On the basis of routine checkup,the intervention group underwent unified comprehensive intervention during pregnancy,including health education,medical nutrition,weight management,medical exercise,and regular self-monitoring on blood pressure and blood sugar.The pregnancy weight gain,labor time,neonatal weight,dystocia rate,and the incidence of maternal and infant complications were compared between the two groups.Results The pregnancy weight gain of the intervention group was significantly less than that of the control group [(9.30±1.32)kg vs.(12.17±2.90) kg,P＜0.001],the labor time of the intervention group was significantly shorter than that of the control group [(10.90±2.29)h vs.(12.82±1.82)h,P＜0.001],the neonatal birth weight of the intervention group was significantly less than that of the control group [(3.24±0.33)kg vs.(3.48±0.32)kg,P＜0.001],the amount of postpartum hemorrhage of the intervention group was significantly less than that of the control group [(270.99±174.16)ml vs.(328.60±108.2 l)ml,P＜0.05];the dystocia rate,the incidences of gestational diabetes and macrosomia,the rate of postpartum hemorrhage of the intervention group were 7.78％,31.11％,2.22％,2.22％,those of the control group were 18.89％,46.67％,11.11％,10.00％,with statistically significant differences (P＜0.05).Conclusion Comprehensive intervention during pregnancy can effectively control the weight of obese pregnant women and reduce the incidence of adverse pregnancy outcomes,such as reducing the incidence of macrosomia,and shorten the labor time,reduce the incidence of dystocia,reduce gestational diabetes and postpartum hemorrhage and other complications,it is worth further publicity and promotion.关键词
