Abstract
Objective To investigate the effect of laparoscopic surgery combined with Gonadotropin-Releasing Hormone Agonist (GnRHa) in the treatment of ovarian chocolate cyst with infertility.Methods A prospective study of 82 patients with ovarian chocolate cysts and infertility who were treated in our hospital from May 2016 to January 2017 was made. All patients were randomly divided into the control group (n=41,treated with laparoscopic surgery) and the observation group (n=41,treated with laparoscopic surgery combined with GnRHa).The clinical effect,reproductive outcome,pre-and posttreatment hormone levels,changes in clinical symptom scores and adverse reactions were observed in the two groups. Results The total effective rate and pregnancy rate in phase Ⅲ-Ⅳ of the observation group were higher than those of the control group,with statistically significant differences (P＜0.05).There was no statistically significant difference between the two groups in pre-treatment hormone levels and symptom scores (P＞0.05),but the post-treatment clinical symptoms scores of the two groups were lower than those before treatment with statistically significant difference (P＜0.05).The posttreatment symptom score of the observation group was lower than that of the control group,with statistically significant difference (P＜0.05).The post-treatment luteinizing hormone (LH) and follicle stimulating hormone (FSH) of two groups were higher than those before the treatment,with statistically significant difference (P＜0.05),and the level of post-treatment estradiol (E2) and progesterone (P) of two groups were lower than before the treatment,with statistically significant difference (P＜0.05).The post-treatment LH and FSH of the observation group were higher than those of control group, and post-treatment E2 and P levels of the observation group were lower than those of control group,both with statistically significant differences (P＜0.05).The adverse reactions of low estrogen level symptoms appeared in patients of the observation group after second medication but were well tolerated,and those with severe symptoms were relieved after treatment.Conclusion Application of combination of laparoscopic surgery and GnRHa in the treatment of ovarian chocolate cyst with infertility can improve the curative effect,the pregnancy rate and ovarian functions,and reduce the postoperative recurrence rate,which is worth promoting.关键词
