心房颤动冷冻消融复发患者的二次手术特点
Characteristics of redoing procedure of patients with recurrence of atrial fibrillation after cryoballoon ablation

Abstract
Objective To explore the characteristics of redoing procedure in patients with recurrence of atrial fibrillation after cryoballoon ablation. Methods One hundred and nineteen consecutive atrial fibrillation patients with cryoballoon ablation from Sept. 2013 to Mar. 2017 were recruited and 11 of them had recurrence of atrial fibrillation and redoing ablation was performed. The characteristics of pulmonary vein-left atrial re-conduction and the characteristics of intraoperative tachycardia were analyzed. Results There were no significant differences of baseline characteristics between patients with recurrence and patients without recurrence of atrial fibrillation. All of the 11 patients accepted the redoing ablation after (6.0±1.3) months. There were altogether 33 (75.0％) pulmonary vein-left atrial re-conduction, including 7 (21.3％) left superior pulmonary vein-left atrial (LSPV), 9 (27.3％) left inferior pulmonary vein-left atrial (LIPV), 6 (18.3％) right superior pulmonary vein-left atrial (RSPV) and 11(33.3％) right inferior pulmonary vein-left atrial (RIPV) re-conduction. The site of re-conduction was difficult to find and the redoing linear-ablation was needed. Three patients needed isolation of superior vena cava. Additional ablation was applied at mitral isthmus line, tricuspid isthmus line and left atrial roofline in 9 patients whose atrial fibrillation was not terminated after isolation of pulmonary vein. Conclusions In patients with recurrence of atrial fibrillation after cryoballoon ablation, the pulmonary vein-atrial re-conduction mostly occurred at left and right inferior pulmonary vein. The site of re-conduction was difficult to identify, and redoing pulmonary vein linear ablation was needed. Some of the patients had extra-pulmonary vein driving sites of tachycardia, needing mapping of the route and performing the ablation.关键词
