4R危机管理在血液透析患者动静脉内瘘维护中的应用
Application of 4R crisis management in the maintenance of arteriovenous fistula in hemodialysis patients

Abstract
Objective To explore the application of 4R crisis theory in the management of arteriovenous fistula in hemodialysis patients.Methods 292 patients undergoing maintenance hemodialysis in our hospital were selected as the subjects from March 2017 to February 2018,and were divided into the control group (144 cases)and the experimental group (148 cases) with the convenient sampling method.The control group received common internal fistula management,the experimental group received 4R crisis management on the basis of common internal fistula management.The incidence of internal fistula complications and hemodialysis adequacy were compared between the two groups.Results The incidence of complications of the experimental group was lower than that of the control group (12.8％ vs.32.6％),with statistically significant difference (P＜0.05).The adequacy of hemodialysis in the experimental group was higher than that in the control group [(1.36±0.20) vs.(1.21±0.18)],with statistically significant difference (P＜0.05).Conclusion The 4R crisis theory can significantly reduce the complications of arteriovenous fistula in maintenance hemodialysis patients,and improve the adequacy of hemodialysis.关键词
