Abstract
Objective:To understand the effects of calcium and phosphorus cognition level on calcium and phosphorus metabolism and parathyroid hormone in maintenance hemodialysis (MHD) patients.Method:A total of 53 MHD patients with dialysis age>3 months treated in Hui Ya Hospital of the First Affiliated Hospital,Sun Yat-Sen University from January 2014 to December 2017 were selected as study objects.The patients' gender,age, dialysis age,primary disease,education level,recent laboratory results calcium and phosphorus metabolism health awareness of the questionnaire were recorded.Result:Among 53 MHD patients,31 cases (58.5%) were males,the average age was (53.2±13.8) years,education level of above senior middle school had 16 cases (30.2%), residual urine volume had 23 cases (43.5%),the average dialysis age was 22.0 (9.0,40.0) months, the average URR was (65.3±6.1)%,the average serum calcium level was (2.24±0.24) mmol/L,the average serum phosphorus level was (2.08±0.61) mmol/L,the average Ca-P product level was (58.4±17.5) mg2/dL2, the average iPTH level was 242.0 (125.5,783.5) ng/L.Among 53 MHD patients,17 cases (32.1%) met the standard of serum phosphorus,29 cases (54.7%) achieved the standard of Ca-P product,and 14 cases (26.4%) achieved the standard of iPTH control.Among 53 patients,43 cases (81.1%) had a score of more than 40 scores (standard group),10 cases (18.9%) had a score of less than 40 scores (non-standard group),and 81.1% had a healthy awareness of calcium and phosphorus metabolism.There were no significant differences in gender, age,education,residual urine volume,dialysis age,URR,serum calcium and iPTH levels between the two groups (P>0.05).However,the blood phosphorus and Ca-P product in the standard group were significantly lower than those in the non-standard group,the differences were statistically significant (P<0.05).Multiple and stepwise linear regression indicated that iPTH was the independent risk factors for hyperphosphatemia (P<0.05). Conclusion:Calcium and phosphorus cognitive level significantly affect serum phosphorus and calcium-phosphate product in maintenance hemodialysis patients.Improving the cognitive level of calcium and phosphorus in maintenance hemodialysis patients is helpful to improve their quality of life and survival rate.The iPTH level is the independent risk factors for hyperphosphatemia.关键词
