Abstract
Objective:To explore the comparison of postoperative analgesia between Dexmedetomidine and Propofol in patients with total hip arthroplasty under combined spinal-epidural anesthesia.Method:A total of 40 patients undergoing total hip replacement under combined spinal-epidural anesthesia from January 2017 to June 2018 in our hospital were selected.According to the random number table method,they were divided into DEX group(D group) and Propofol group(P group),20 cases in each group.D group received continuous intravenous infusion of DEX at 0.3-0.5 μg/(kg·h) after combined spinal-epidural anesthesia,and P group received continuous intravenous infusion of Propofol at 0.8-2.0 mg/(kg·h) after combined spinal epidural anesthesia.The heart rate(HR),mean arterial pressure(MAP) and pulse oxygen(SpO2) at different time after anaesthesia were recorded,the VAS scores of 5 h(H0),10 h(H1),15 h(H2),24 h(H3),36 h(H4) and 48 h(H5) after operation were compared between two groups,and postoperative adverse reactions were recorded. Result:The HR, MAP and SpO2at different time after anaesthesia in two groups were compared,the differences were not statistically significant(P>0.05).The VAS scores of H0,H1 and H2 in D group were lower than those of P group,the differences were statistically significant(P<0.05),but the VAS scores of H3,H4 and H5 in two groups were compared,the differences were not statistically significant(P>0.05).The number of postoperative nausea,vomiting,drowsiness and sweating in D group were higher than those of P group,but the number of postoperative delirium was less than that of P group,the differences were statistically significant(P<0.05). Conclusion:There is no difference in the effects of DEX and Propofol on total vital signs in patients undergoing total hip arthroplasty under combined spinal-epidural anesthesia.After surgery,DEX has a longer analgesic effect than Propofol,and can reduce postoperative delirium.关键词
