Abstract
Objective To explore the relationship between brain symmetry index ( BSI) and the severity and prognosis of patients with severe cerebrovascular disease. Methods Fourty-eight patients with severe cerebral vascular disease accompanied with consciousness disorder were selected. The EEG was detected within 3 days after disease or aggravation. The GCS, NIHSS and acute physiology and chronic health evaluationⅡ(APACHEⅡ) score were recorded. Twenty-one patients were detected by EEG dynamic monitoring at 7 d and 14 d after disease or aggravation, and the prognosis was evaluated by GOS at 3 months. Results BSI at each time points between cerebral infarction patients and cerebral hemorrhage patients had statistical significance ( all P<0. 001 ). BSI of cerebral infarction patients at 7 d was significantly higher than that at 3 d and 14 d ( all P<0. 05 ), BSI of cerebral hemorrhage patients at 3 d was significantly higher than that at 7 d and 14 d (all P<0. 05). The slow wave index, delta waves and theta power in affected side of cerebral infarction patients at 7 d group were significantly lower than those in healthy side (all P<0. 05), the α wave exponent and theta wave power in affected side of cerebral infarction patients at 14 d were significantly higher than those in healthy side (all P<0. 05). The slow wave index, delta waves and theta power in affected side of cerebral hemorrhage patients within 3 days were significantly higher than that in healthy side (all P<0. 05), and the α wave exponent in affected side of cerebral hemorrhage patients at 14 d was significantly lower than that in healthy side (P<0. 05). BSI had positive correlation with absolute value of bilateral slow wave index power difference (t=6. 524, P<0. 001). The univariate analysis showed that BSI, absolute value of the theta power between the healthy side and the affected side, GCS, NIHSS and APACHEⅡ score had influence on the prognosis of the patients (all P<0. 05). Multivariate analysis showed that BSI, GCS had an impact on the prognosis (all P<0. 05). Conclusion BSI can reflect relatively accurate changes of brain function in patients with severe cerebrovascular disease, and it can provide guidance for treatment and predicting prognosis.关键词
