Abstract
Objective To investigate the diagnotic value of 3-nitro-tyrosine ( 3-NT ) for minimal hepatic encephalopathy (MHE). Methods Ninety epatitis B cirrhotic patients without overt hepatic encephalopathy (OHE) (liver cirrhosis group ) and 90 normal controls ( normal control group ) measured by psychometric hepatic encephalopathy score (PHES). The serum levels of 3-NT, IL-6, IL-18, methionine and citrulline were determined. The PHES was used as gold standard, the sensitivity and specificity were evaluated. Results According to PHES, 39 patients were diagnosed to be MHE. Compared with those in non-MHE subgroup, the levels of 3-NT, IL-6, IL-18, methionine and citrulline in MHE subgroup were significantly increased ( all P<0. 01). Logistic regression analysis showed that 3-NT ( P=0. 005), IL-6 ( P=0. 029) and IL-18 ( P=0. 018) were related risk factors of MHE, whereas citrulline ( P=0. 087 ) and methionine ( P=0. 401 ) didn’t have predictive value to detect MHE. The receiver operating characteristic curve analysis showed 3-NT had high value in diagnosis of MHE, the area under curve of MHE was 0. 967, the cutoff point was 13. 230 nmol, the sensitivity was 92. 5% , specificity was 94. 0% . Conclusion 3-nitro-tyrosine has a good role to detect MHE, which is easy and not time consuming, with good sensitivity and specificity.关键词
