Abstract

Objective To investigate the drug therapy of Kawasaki disease (KD),to evaluate the sensitivity and specificity of the four commonly used scoring systems in predicting KD complicated with coronary artery injury (CAL),and to select the most suitable scoring system.At the early stage of the disease,high-risk patients were screened out for active treatment and strict follow-up was provided.Methods The medical records of the children diagnosed with KD at Suzhou Municipal Hospital were analyzed retrospectively.According to the inclusion criteria,the children were divided into a non-coronary artery injury (NCAL) group and a coronary artery injury (CAL) group.The drug treatment status of these children was investigated.Then four kinds of scoring systems were used to evaluate the children with KD.Mann-Whitney U rank sum test was used to analyze the discriminating ability of the four models for the CAL group and the NCAL group,and ROC curve was used to evaluate the predictive value of risk assessment model.Results 21 cases (23.60%) were included in the CAL group,and 68 cases (76.40%) in the NCAL group.87 cases (97.75%) received intravenous immunoglobulin (IVIG) and 2 cases (2.25%) did not use IVIG and had CAL.85 cases (95.50%) were treated with aspirin (ASA),4 cases (4.50%) were not treated with ASA,and one case had coronary artery injury.The results of Mann-Whimey U rank sum test showed that the four risk assessment models had statistical differences in risk grade data between the two groups (P < 0.05),and all of them could distinguish whether the children were complicated with CAL or not.ROC curve showed that the score of Chen Jingjing had higher sensitivity and specificity.Conclusion Appropriate scoring system can assist the clinical screening of high-risk children at an early stage and give them active treatment,so as to reduce the risk of CAL.
