以神经为导向的TME与传统TME对中青年男性泌尿生殖功能影响的对比
Comparative study of neuro-oriented TME and traditional TME on urogenital function in young or middle-aged male patients

Abstract
Objective To compare the efficacies of neuro-oriented total mesorectal excision (TME) and traditional TME on the genitourinary function of rectal cancer patients, thus to investigate the protective effects of neuro-oriented TME on pelvic autonomic nerves. Methods 58 young or middle-aged male patients wiht laparoscopic TME were retrospectively analyzed, among whom 32 cases received laparoscopic neuro-oriented TME (observation group) and 26 cases recevied conventional laparoscopic TME (control group). The intraoperative related indexes, urinary function at the 1st month, 3rd month, 6th month and 12th month after surgery, and ejaculation and erectile function at the 3rd month, 6th month and 12th month after surgery were compared and analyzed between the two groups. Results There was no significant difference in intraoperative indexes between the two groups (P＞0.05). Compared with control group, the rates of urinary dysfunction in the observation group were significantly lower than that in the control group at the 1st month, 3rd month and 6th month after operation (P＜0.05), but there was no statistical significance in micturition function between the two groups at the 12th month after operation (P＞0.05). The rates of erectile and ejaculation dysfunctions were significantly reduced in the observation group at the 3rd month, 6th month and 12th month after operation (P＜0.05). Conclusion The neuro-oriented TME is better finding the safe cutting layer, reducing the injury to the pelvic autonomic nerves during operation, thus reduces the incidence of postoperative urinary and sexual dysfunction in young or middle-aged male patients with rectal cancer.关键词
