Abstract
Objective Through an example of surgical treatment for an infectious endocarditis patient with acute coronary syndrome complicated with chronic renal insufficiency, this paper explores how to balance the safety of perioperative revascularization in patients with high risk of thrombosis and hemorrhage, and carries out antithrombotic, anti-infective and pharmaceutical care to improve the safety and effectiveness of surgery and drug therapy. Methods The patient had hypertension, diabetes, diabetic nephropathy and other basic diseases. At present, CKD5 long-term dialysis treatment, this time due to acute coronary syndrome and infectious endocarditis admission, coronary artery bypass grafting and valve replacement will be performed with high risk of bleeding and thrombosis. There was a contradiction between thrombotherapy. Perioperative revascularization safety management and pharmaceutical care should be carried out for these patients. Clinical pharmacists used pharmacy knowledge, combined with laboratory tests and imaging data, and worked out drug treatment plans with doctors, carried out pharmaceutical care throughout the process, paid attention to adverse drug reactions and interactions, and monitored the concentration of vancomycin and INR value, as well as educated patients about drug use. Results The patient's infection was effectively treated and discharged from hospital. Conclusion Clinical pharmacists can provide safe and effective pharmaceutical care for patients by providing pharmaceutical care to improve the level of clinical drug treatment.关键词
