Abstract
Objective To investigate the relationship between maternal obesity and gestational diabetes mellitus and risk of neonatal hypoglycemia. Methods Retrospective analysis was conducted on 120 cases of gestational diabetes mellitus and neonates in Taizhou Hospital of Zhejiang Province from January 2015 to February 2017. According to neonatal hypoglycemia, neonates were divided into hypoglycemia group (n=73) and blood glucose normal group (n=47). The clinical data of two groups of neonates were compared. ROC curve was used to analyze the relationship between neonatal hypoglycemia and maternal obesity as well as gestational diabetes mellitus. Influencing factors of neonatal hypoglycemia in cases of gestational diabetes mellitus was explored by Logistic regression analysis. Results There are significant differences in birth weight (t=5.012) , maternal obesity (χ2=6.516) , maternal BMI (t=5.084) , preterm infant (χ2=4.984) , mode of delivery (χ2=5.312) between hypoglycemia group and blood glucose normal group (all P<0.05). Diagnostic efficiency was highest when maternal BMI index was 24.3 kg/m2 (AUC=0.845, Z=5.873, P=0.007) , and the sensitivity of predicting neonatal hypoglycemia by gestational diabetes mellitus was 82.4％ and specificity was 85.6％. Multivariate Logistic regression analysis showed that obesity was an independent risk factor for neonatal hypoglycemia in gestational diabetes mellitus (OR=1.609, 95％CI:1.342-2.580). Conclusion Obesity is an independent risk factor for neonatal hypoglycemia in gestational diabetes mellitus. Controlling maternal weight or early intervention can reduce the incidence of neonatal hypoglycemia in gestational diabetes mellitus.关键词
