Abstract
Objective To investigate the safety and efficacy ofusing perfusion weighted imaging (PWI) to guide intravenous thrombolysis in WUS patients last-seen-well more than 4. 5 hours.Methods Data on all AIS patients with WUS was reviewed.Of which, 36 patients had ischemic penumbra using PWI and last-seen-well more than 4. 5 hours.The control group consisted of those patients treated within 4. 5 hours.They all were treated with intravenous recombinant tissue plasminogen activator thrombolysis.Data elements collected and analyzed included: demographics, National Institutes of Health Stroke Scale (NIHSS) score at baseline and at 2h, 24 h, 7d after thrombolysis and before discharge, the modified Rankin Scale (mRS) at 3 months after discharge, imaging findings and any complication.Results The NIHSS score showed decrease in each time point after thrombolysis in both groups.The NIHSS scores at 2h, 24 h, 7d after thrombolysis and/or before discharge in WUS group were significantly improved compared to before t PA (P＜0. 05).The NIHSS score at 7d after thrombolysis and before discharge in the control group also improved significantly (P＜0. 05).Between the two groups, no significant differences in mRS scores (0-2) at 3 months (P＞0. 05) was observed.Their incidence of adverse events after thrombolysis between the two groups were similar.Conclusion Utilizing PWI to guide intravenous thrombolysis for WUS patients with last-seen-well＞4. 5h was safe and effective.关键词
