孕期铁摄入与妊娠期糖尿病相关性的Meta分析

Abstract
Objective To systematically assess the correlation between iron intake during pregnancy and gestational diabetes mellitus (GDM) risk.Methods The databases of PubMed, Web of Science, Cochrane Library, CNKI and Wanfang databases were retrieved by computer to search the literatures about the correlation between GDM and iron supplementation at home and abroad.The retrieval time was from the database establishment to November 2017.The two individuals independently conducted the literature screening according to the inclusion and exclusion criteria, data extraction and data evaluation.Then the STATA 14.0 and RevMan 5.3 softwares were used to conduct the Meta-analysis.Results A total of 8 articles meeting the inclusion criteria, including 25 970 participants and 2 213 cases of GDM.The meta analysis results showed that the dietary intake of heme iron was significantly correlated with the risk of GDM (RR=1.76, 95％CI:1.40-2.19) and there was no significant correlation between the GDM risk and the pregnant women with dietary non-heme iron supplementation (RR=0.93, 95％CI:0.77-1.14) .There was no significant correlation between pregnant women and GDM risk in the prospective cohort study of the iron preparation supplement experiment (RR=1.75, 95％CI:0.56-5.47) , and there was no significant correlation between the pregnant women with iron preparation supplement and GDM risk (RR=0.88, 95％CI:0.72-1.07) .The women with high total iron intake had no significant correlation with the risk of GDM (RR = 1.10, 95％ CI:0.76-1.57) .Conclusion Increasing dietary heme iron is positively correlated with the risk of developing GDM in pregnant women.No association of GDM with dietary non-heme iron, iron supplementation, and total iron intake is detected and these will help the future studies to understand the role of iron in GDM development.At the same time, it is suggested that different pregnant women should formulate the personalized iron supplementation strategies during pregnancy.关键词
