血小板减少患儿血小板抗体筛查及分析
Screening and analysis of platelet antibodies in children with thrombocytopenia

Abstract
Objective To explore the influencing factors of platelet antibody production in pediatric patients, further provide evidences for the diagnosis and therapy for children with platelet antibody associated diseases. Methods The solid phase agglutination was applied to detect platelet antibodies in samples acquired from 1409 children suffering from thrombocytopenia in Beijing Children's Hospital during December 2016 to May 2017. The demographic characteristics, ABO blood group and disease distribution of platelet antibody positive children were analyzed. Results The positive rate of platelet antibody presence in children with thrombocytopenia was 12.49% (176/1 409), of which strong positive accounted for 64.77% (114/176) and weak positive 35.23% (62/176); the proportion of platelet antibody negative was 87.51% (1 233/1 409). The positive rate of platelet antibody in children showed no significantly difference between gender or ABO blood group distribution (P>0.05). The positive rate of platelet antibody in infants (aged 29 d-2 years old) [17.38% (81/466) ] and children (aged>2-6 years old) [14.44% (41/284) ] were higher than that in children of other ages. The positive rates of platelet antibody in clinical departments were different (P<0.01). The positive rates of platelet antibody in the Otolaryngology [22.22% (2/9) ], Rheumatology[20.00% (3/15) ], Infection[25% (2/8) ], Respiratory[20.00% (2/10) ], Dermatology[20.00% (3/15) ], ENT[17.78% (8/45) ], Gastroenterology[37.04% (10/27) ] and Oncology[25.00% (3/12) ] were higher than those in other clinical departments (P<0.05). The positive rate of platelet antibody in acute lymphoblastic leukemia [17.29% (23/133) ], neuroblastoma [25.00% (7/28) ], immune thrombocytopenia [42.86% (3/7) ] and cholestatis liver disease [60.00% (3/5) ] were significantly higher than that of other diseases [11.47% (142/1238) ] (P<0.05), respectively. Conclusions The infants and young children (aged <6 years old) with thrombocytopenia are prone to develop platelet antibody. The acute lymphoblastic leukemia, neuroblastoma, immune thrombocytopenia and cholestatic liver disease may be risk factors for platelet antibody production in pediatric patients.关键词
