Abstract
Objective:To establish a hospital prescription (including ward medication orders) reviewing system, and strengthen the prescription audit pre-intervention ability to improve the rational use of drugs.Method:According to the Joint Commission International (JCI) hospital accreditation standards, a hospital prescription review system was developed.The prescription was checked and intercepted by the double mode of software automatic audit and manual audit.Result:In 2017, there were 160 494 medical orders in all hospitalized patients in our hospital, the rate of intervention by pharmacists for irrational prescriptions was 8.63％, the rate of dismissal was 0.65％, and the reasonable rate of medical advice was increased month by month.Conclusion:Successful establishment of the prescription suitability audit system has realized the pre-intervention of drug use, interception of irrational drug use and improvement of drug safety.However, the review system still needs to be continuously improved in details such as the review of chemotherapy programs, and strengthen the professional level of pharmacists to ensure the safety of each patient's medication.关键词
