Abstract
Objective To investigate whether NIHSS/DWI-ASPECTS mismatch can predict early neurological dete-rioration in patients with middle cerebral artery cerebral infarction. Methods The hospitalized patients with middle cere-bral artery cerebral infarction in the Department of Neurology,Second Affiliated Hospital of Anhui Medical University from March 2017 to May 2018 were retrospectively collected. The clinical data were collected and NIHSS scores was evaluated at admission and 72 hours after hospitalization to clarified early neurological deterioration (END) and to divided patients into END group and non-END group. DWI-Alberta Stroke Project Early CT Score ( DWI-ASPECTS) was measured within 48 hours at admission,NIHSS/DWI-ASPECTS mismatch ( NDM) was defined as NIHSS score≥8 and DWI-ASPECTS≥8;NDM was analyzed by multivariate logistic regression to analyze the correlation between NDM and END in middle cerebral artery cerebral infarction. Results A total of the 94 patients with middle cerebral artery cerebral infarction were collected, including 52 males (55. 3% );age (65. 4 ± 11. 0) years. 31 patients (33% ) were eligible for END. There were 18 NDM cases (19. 1% ),including 14 cases (45% ) in END group and 4 cases (6. 5% ) in non-END group. There were statistic difference in age,NIHSS,DWI-ASPECTS,NDM between END and non-END group (P≤0. 05 or P≤0. 01). Multivariate logistic regression analysis showed that DWI-ASPECTS ( odds ratio 0. 637,95% confidence interval 0. 409 ～0. 994,P=0. 047) and NDM (odds ratio 13. 175,95% confidence interval 1. 539～112. 824,P=0. 019) were predictors of END in symptomatic middle cerebral artery stenosis after adjusting for confounding factors. Conclusion NIHSS/DWI-ASPECTS mismatch may have a certain predictive value for early neurological deterioration in patients with middle cerebral artery cere-bral infarction.关键词
