Surgical treatment methods of Helveston syndrome with congenital nystagmus

Abstract
Objective To investigate the surgical treatment methods and the postoperative results of Helveston syndrome with congenital nystagmus. Methods 13 cases with Helveston syndrome with congenital nystagmus were studied. The surgical designs first were considered to correct anomalous head position angles with the middle zone of nystagmus and A-pattem exotropia, then the degree of superior oblique muscle overaction and dissociated vertical deviation (DVD). After the operation, the patients' anomalous head position angle, ocular position, nystagmus and binocular distance visual acuity were observed. Results Of 12 (92.3％) cases had eliminated anomalous head position angle, In all cases, A-pat-tem exotropia was corrected, the symptoms of the nystagmus were markedly reduced, and the visual acuity got much more improvement in different degrees. Of 2 (15.4％) cases got stereoscopic vision of far and near, 7 (53. 8％) cases had e-liminated DVD, 6 (46. 2％) cases markedly reduced DVD who did not need surgery. There were obvious differences in horizontal strabismus between preoperative and postoperative that were statistically significant (t = -9. 551, P = 0. 000); There were obvious differences in A syndrome between preoperative and postoperative that were statistically significant (t = 11.094, P = 0.000); There were obvious differences in DVD between preoperative and postoperative that were statistically significant (t = 5. 828, P = 0. 000). Conclusions The surgical treatment of Helveston syndrome with congenital nystagmus first should correct anomalous head position angle and A-pattern exotropia, and then considered the hyperactivity of the superior oblique muscle and the correction of DVD. The individualized surgical design was the precondition of obtaining good results and reducing the reoperation.关键词
