Abstract
Objective Anterior commissure of glottic laryngeal cancer has always been a difficult problem in self-retaining laryngoscopy surgery. Some patients' anterior commissure can not be exposed or can not be cut accurately after exposure. This paper discusses the effect of CO2 laser combined with Peng's multifunctional surgical dissector (after modification) guided by nasal endoscopy in the operation of glottic laryngeal cancer involving anterior commissure. Methods The clinical data of 20 patients with glottic laryngeal cancer admitted to our department from March, 2016 to December, 2017 were retrospectively analyzed. The clinical effects of CO2 laser combined with endoscopic Peng's multifunctional surgical dissector (after modification) were evaluated. Result In the 20 cases of glottic laryngeal cancer involving anterior commissure, Peng's multifunctional surgical dissector guided by CO2 laser combined with nasal endoscopy was used to complete the removal of the tumors. The safety margin was accurate. All the cases were followed up so far without recurrence, tracheotomy, aspiration, and dyspnea. Bilateral vocal cords were not viscous after operation. Conclusion In self-retaining laryngoscopy for glottic cancer involving anterior commissure, nasal endoscopy combined with Peng's multifunctional surgical dissector can widen the vision field of tubular surgery, expose the combined tumors before exposure, make up for the limitation of the combined tumors before CO2 laser resection by using the flexibility of attracting electrocoagulation, and accurately resect the combined tumors before CO2 laser resection under the condition of ensuring the safe margin. It is an ideal method for the treatment of pre-invasive anterior commissure of T1 and T2 glottic carcinoma, and has clinical value.关键词
