腰硬联合阻滞麻醉对老年人下肢骨折手术麻醉效果的研究
Effects of Combined Spinal-epidural Anesthesia on Lower Limb Fracture Surgery in Older Patients

Abstract
Objective To assess the effects of combined spinal-epidural anesthesia on older patients with lower limb frac-ture undergoing surgery. Methods 228 older patients with lower limb fracture undergoing surgery were randomly divided into the ob-servation group and the control group. The control group was given epidural anesthesia, and the observation group was given combined spinal epidural anesthesia. Intraoperative anesthesia, operative related indicators, respiratory function and pulmonary infection were recorded. The sedative effect and cognitive function in patients were evaluated. Results There were no significant differences in vis-ual analogue score (VAS), intraoperative blood loss, duration of the operation, the time of hospitalization between the two groups (P>0.05). The time of anesthesia, the dosage of drugs, the time of complete block and the time of recovery after anesthesia in the observation group were better than those in the control group (P<0.001). The respiratory rate (RR), oxygen saturation (SpO2), minute ventilation (MV) in the observation group were better than those in the control group (P<0.05). The incidence of postoper-ative pulmonary infection in the observation group was lower than that in the control group (P<0.05). The sedative effect 10 min and 30 min after surgery in the observation group were better than those in the control group (P<0.001). The mini-mental state exami-nation (MMSE) scores 12 h and 24 h after anesthesia in the observation group were higher than those in the control group ( P <0.01). Conclusion The anesthesia and sedation effect of combined spinal epidural block anesthesia are better than those of epidural anesthesia, the incidence of postoperative pulmonary infection is lower, and the recovery of postoperative cognitive function is faster.关键词
