Abstract
Objective To evaluate the significance of dynamic monitoring of serum cholinesterase(ChE),preal﹣bumin(PA)and blood glucose levels in the condition evaluation and prognosis of patients with severe pneumonia. Methods 5 1 patients with severe pneumonia were divided into death group(n=14)and survival group(n=37)ac﹣cording to different prognosis.87 patients with common pneumonia and 69 healthy people with outpatient health exami﹣nation were selected as control groups in the same period.Serum levels of ChE,PA and blood glucose were compared among these groups.The correlation between above indicators and the prognosis of patients with severe pneumonia was analyzed.Results Serum levels of ChE and PA in severe pneumonia group were lower than those in common pneu﹣monia group,serum levels of ChE and PA in common pneumonia group were lower than those in control group,levels of blood glucose in severe pneumonia group were higher than those in common pneumonia group,and levels of blood glucose in common pneumonia group were higher than those in control group(P<0.05).The serum levels of ChE and PA in survival group were higher than those in death group on the first day and the third day,and the level of blood glucose in survival group at the same time was lower than that in the death group (P<0.05).On the first day and the third day after admission,serum levels of ChE and PA in the survival group showed an upward trend and a correspond﹣ing drop in blood glucose,while serum levels of ChE and PA in the death group showed a downward trend and a corre﹣sponding rise in blood glucose(P<0.05).Cox proportional hazard model analysis showed that ChE and PA were pro﹣tective factors for prognosis of patients with severe pneumonia,and blood glucose was a risk factor for prognosis of pa﹣tients with severe pneumonia.ROC curve showed that the area under the ROC curve evaluated by ChE for prognosis of patients with severe pneumonia was 0.824,PA was 0.753,blood glucose was 0.705,and ChE+PA+blood glucose was 0.903.Conclusion Serum levels of ChE,PA and blood glucose can reflect the severity of severe pneumonia,and have a high predictive value for the prognosis of patients.关键词
