早发型与迟发型新生儿败血症临床指标及病原菌分析
Analysis of clinical parameters and pathogenic bacteria in early-onset and late-onset neonatal sepsis

Abstract
Objective To analyze the clinical characteristics,distribution of the pathogens,and drug sensitivity test results between early and late onset neonatal sepsis patients,and to support diagnostic and empirical treatment for neonatal sepsis.Methods A total of 372 neonatal sepsis patients were enrolled in the study,and 174 neonates of them were early onset,198 neonates were late onset.The clinical characteristics and distribution of the pathogens were compared between early and late onset group.Results Born body weight,and apgar scores at 1and 5 minute and the mean value of white blood cells and lymphocytes from early onset group were higher than late onset group(P<0.05),but the rates of preterm and patients with CRP raised,the proportion of mortality and the average of PLT were higher in late onset group(P<0.5).Gram-positive bacteria accounted for 71.84% and 57.58% in the pathogens of early and late onset sepsis group respectively,of which coagulase-negative staphylococci were predominant.While E.coli and Klebsiella pneumoniae were main gram-negative bacteria.The drug sensitivity test results of gram-positive cocci were no significant different between the two groups.However,the gram-negative bacteria from late onset group had higher resistance rates to ampicillin,cefotaxime,ceftazidime,and ceftriaxone than early onset group (P<0.05).Conclusions Coagulase-negative staphylococci,E.coli,and Klebsiella pneumoniae were the top 3 pathogens of neonatal sepsis in our hospital.And the clinical characteristics,distribution of the pathogens,and drug sensitivity test results were different between early and late onset neonatal sepsis cases,clinical diagnosis and antibacterial drugs could be conducted according to our study.关键词
