缺血性脑卒中患者血清中miR-191 、miR-18 a的表达与预后关系
The relationships between the expressions of serum miR-191 and miR-18a and prognosis in patients with ischemic stroke

Abstract
Objective To explore the expression levels of microRNA-191 (miR-191) and microRNA-18a ( miR-18a) in the serum of patients with ischemic stroke and their relationships with prognosis. Methods 118 patients with is-chemic stroke treated in our hospital were selected as the study subjects ( stroke group),and 114 healthy people were se-lected for the comparative study (health group). The serum levels of miR-191 and miR-18a in all subjects were detected, the single and combined diagnostic value of miR-191 and miR-18a for ischemic stroke was analyzed,the levels of miR-191, miR-18a and MRS scores of different prognosis were analyzed,and the prognostic factors of ischemic stroke were analyzed. Results Compared with the healthy group,the number of diabetes mellitus patients and the expression level of miR-191 in stroke group increased (P<0. 05),while the expression level of miR-18a decreased (P <0. 05);the area under curve ( AUC) of serum miR-191 and miR-18a for the diagnosis of ischemic stroke was 0. 878 and 0. 765 respectively,and the cut-off values were 1. 28 and 0. 86 respectively,the AUC of the combined diagnosis of ischemic stroke was 0. 907;compared with the group with good prognosis,the expression level of miR-191 and MRS score were higher in the group with poor prog-nosis (P<0. 05),while the expression level of miR-18a was lower in the group with poor prognosis (P<0. 05);history of diabetes mellitus,miR-191 and MRS score were risk factors for prognosis of ischemic stroke (P<0. 05),and miR-18a was prognostic protective factor (P<0. 05). Conclusions The serum level of miR-191 is high and miR-18a is low in patients with ischemic stroke,both of them may be involved in the pathogenesis of ischemic stroke,and the combined detection of miR-191 and miR-18a can evaluate the prognosis of ischemic stroke.关键词
