颈源性头痛针刀治疗靶点体表定位及其分布规律
Surface location and distribution of acupotomology target for cervicogenic headache

Abstract
Objective The diagnosis and treatment information of cervicogenic headache patients were analyzed to study the surface location and distribution of acupotomology target.Methods Patients who were initially diagnosed as cervicogenic headache found positive reaction points by following methods:1.the pain site complained by the patients;2.surface projection of the high cervical nerve (C1-C3);3.myofascial tissue and its fascial chain innervated by the high cervical nerve.After local anaesthetic blockage effect was positive at positive reaction points,acupotomology therapy was performed once a week for 3 times.The patients were followed up for 2,6,and 12 weeks after treatment respectively.The clinical effects of headache relief,cervical vertebra mobility,and accompanying symptoms improvement were evaluated,and the surface location and tissue structure of effective treatment points (targets) were analysed,and their distribution patterns were discussed.Results 1.119 cases were diagnosed by cervicogenic headache.The total effective rates were 93.3％ (111 cases),94.9％ (113 cases),and 86.6％ (103 cases) after 2,6,and 12 weeks of followed-up.2.Accuracy and frequency of target location:in the cases of cervicogenic headache,78.2％ (93 cases) of the treatment targets appeared in the midpoint of the line between the C2 spinous process and the mastoid tip,C2-C3 facet joints accounted for 75.6％ (90 cases),the C1 transverse process tip of the inferior margin of the mastoid process accounted for 65.5％ (78 cases);the inner 1/3 and the outer 1/3 of the line between the occipital trochanter and the mastoid process accounted for 52.1％ (62 cases) and 43.7％ (52 cases),the midpoint of the sternocleidomastoid muscle posterior margin accounted for 35.3％ (42 cases),and the rest appeared in the 2-3 cm area beside the spinous process of C3-C7,subcutaneous fascia,superior trapezius muscle,and sternocleidomastoid muscle area.Conclusions Acupotomology therapy targets for cervicogenic headache aremainly located in the greater occipital nerve,the lesser occipital nerve,the third occipital nerve,and the greater auricular nerve,which are easily entrapped by the myofascial fascia,followed by the posteromedial branch of the middle and lower cervical nerve/facet joint.Acupotomology release of stimulus points on myofascial chain can also effectively relieve cervicogenic headache.关键词
