喉返神经监测技术在复杂甲状腺手术中的应用初探*
A Probe into the Application of Intraoperative Neuromonitoring in Com-plex Thyroid Surgery

Abstract
Objective: To discuss the application of intraoperative neuromonitoring (IONM) in complex thyroid or para-thyroid surgery. Methods: A total of 123 patients who underwent complex thyroid or parathyroid surgery in Jiangsu Cancer Hospital were assigned to experimental group (n=63) and control group (n=60). Age, gender, pathological type, history of tracheotomy, incidence of new recurrent laryngeal nerve injury and dyspnea in two groups were analyzed. Results: Incidence of temporary recurrent laryngeal nerve injury were 3. 2% in experimental group and 15% in control group (P=0. 048). Incidence of permanent recurrent laryngeal nerve injury were 0% in experimental group and 3. 3% in control group (P=0. 455). Meanwhile, fewer patients underwent tracheotomy in the IONM group (0 vs 3). Conclusion: IONM can reduce the incidence of recurrent laryngeal nerve injury by accurately locating recurrent laryngeal nerves, and precisely judge the function of recurrent laryngeal nerves to avoid unnecessary trache-otomy and reduce the risk of dyspnea caused by conservative observation.关键词
