锁定钢板与PFNA治疗老年骨质疏松性股骨粗隆间骨折的临床研究

Abstract
Objective To evaluate the clinical efficacy of locking plate and PFNA in the treatment of senile patients with osteoporotic intertrochanteric fracture.Methods From January,2015 to January,2018,112 elderly patients with osteoporotic intertrochanteric fracture of femur were retrospectively analyzed.57 patients were treated with proximal femoral locking plate (group A),and 55 with PFNA (group B).There were 33 males and 24 females as well as 27 type Ⅱ and 30 type Ⅲ of Jensen classification in group A;they were (66.4±4.1) years old.There were 30 males and 25 females as well as 24 type Ⅱ and 31 type Ⅲ of Jensen classification in group B;they were (67.7±3.8) years old.The follow-up time after operation was (16.5±1.4) months.The operation time,the bleeding volume,drainage volume,fracture healing,Harris score,bone mineral density,and incidence of complications were compared between these two groups.Results All the 112 cases were followed up.There were no statistical differences in gender,age,Jensen classification,and time from injury to operation between the two groups (all P ＞ 0.05).The operation time was longer and the bleeding and drainage volumes were less in group A than in group B (all P ＜ 0.05).Group B could obtain early functional exercise and better early fracture healing rate than group A (both P ＜ 0.05).There were no statistical differences in fracture healing rate and incidences of complications12 months in after the operation as well as Harris score between these two groups (all P ＞ 0.05).The bone mineral density was better after than before the operation in both groups (both P ＜ 0.05).There were no statistical differences in the bone mineral density before and after the operation between the two groups (both P ＞ 0.05).Bone healing was achieved in the patients with screw fracture and fracture displacement after revision.Conclusions PFNA for senile patients with osteoporotic intertrochanteric fracture of femur has the advantages of short operation time,less bleeding volume,and high early bone healing rate.关键词
