基于倾向性匹配对进展期胃癌病人根治性手术联合腹腔热灌注化疗安全性的评估
EVALUATION OF SAFETY OF RADICAL SURGERY COMBINED WITH INTRAPERITONEAL HYPERTHERMIA IN PATIENTS WITH ADVANCED GASTRIC CANCER BASED ON PROPENSITY SCORE MATCHING

Abstract
Objective To investigate the perioperative safety of radical surgery combined with intraperitoneal thermal perfusion(HIPEC)for advanced gastric cancer patients.Methods A retrospective analysis was conducted on the clinical data of 94 patients with advanced gastric cancer who underwent radical gastrectomy at the Gastrointestinal Department of the Affiliated Hospital of Qingdao University from January to December 2018.Among them,53 patients underwent simple radical surgery(con-trol group)and 41 patients underwent radical surgery combined with HIPEC(observation group).The postoperative clinical indica-tors,postoperative complications,and other adverse events were compared between the two groups.Propensity score matching was used to adjust for confounding factors.Results A 1∶1 PSM matching showed no significant difference in baseline data between the two groups(P＞0.05).There were no significant differences between the two groups after PSM in terms of surgical time,int-raoperative bleeding volume,and hospital stay(P＞0.05).Comparison of postoperative laboratory indicators showed no significant differences in platelet and urea nitrogen(P＞0.05)but significant differences in white blood cells,creatinine,and postoperative al-bumin(t=2.557-3.842,P＜0.05).There was no significant difference in the incidence of postoperative complications(P＞0.05).There was no significant difference in the incidence of adverse events during the perioperative period among patients receiving 1,2,and 3 rounds of HIPEC treatment(P＞0.05).Conclusion The combination of radical surgery and HIPEC in patients with ad-vanced gastric cancer does not significantly increase the incidence of postoperative complications,and the procedure is safe and feasi-ble.However,it is still necessary to pay attention to the changes in renal function and albumin level in patients after surgery.关键词
