负载rh-BMP2的羟基磷灰石人工骨治疗胫骨平台骨折骨缺损的临床疗效研究
Clinical efficacy of rh-BMP2-loaded hydroxyapatite artificial bone for the treatment of tibial plateau fracture defect

Abstract
Objective To evaluate the clinical efficacy of rh-BMP2-loaded hydroxyapatite(HA)prosthetic material in the repair of tibial plateau fractures in comparison with HA bioceramic prosthetic material.Methods A retrospective analysis was performed on 53 patients with Schatzker type Ⅳ and Ⅵ tibial plateau fractures admitted to department of orthopedics,the 920th Hospital of the Joint Logistics Support Force of PLA from October 2014 to September 2019.All patients underwent open reduction and internal fixation and intraoperative bone grafting.Among them,27 patients were treated with hydroxyapatite artificial bone loaded with rh-BMP2(study group),and the other 26 patients were treated with hydroxyapatite bioceramic bone grafting(control group).The fracture healing time,HSS score,Rasmussen and complications were used to compare the clinical efficacy of the two groups of materials.Results All the patients were followed up for(10.45±2.31)months and all patients finally achieved bone union.The fracture healing time of the study group[(13.46±3.38)weeks]was significantly shorter than that of control group[(16.67±6.51)weeks](P<0.05).The HSS score of the study group[(85.53±5.30)scores]was better than that of control group[(80.58±6.40)scores]at 6 months after operation(P<0.05).However,there was no significant difference in Rasmussen imaging score at 6 months after operation between the two groups(P>0.05).At the same time,there was no significant difference in the incidence of complications(fracture-related infection,fracture recollapse,delayed bone union,joint stiffness)between the two groups(P>0.05).Conclusion Compared with conventional artificial bone in the treatment of tibial plateau fracture defect,hydroxyapatite artificial bone loaded with rh-BMP2 has a shorter fracture healing time and is more conducive to the early recovery of knee joint function,but without increasing the risk of complications.It can be considered as a good alternative material for bone repair.关键词
