Abstract
Objective To study the diagnostic value of neutrophil gelatinase-associated lipocalin(NGAL)in early-stage diabetic nephropathy(DN).Methods A total of 82 patients with early-stage DN admitted to Gao'an People's Hospital from January 2020 to January 2023 were set as DN group,and 50 patients with simple diabetes were set as DM group.The NGAL level,urinary albumin/creatinine ratio(UACR)and glomerular filtration rate(eGFR)of the two groups were detected,and the diagnostic value of NGAL index for early DN was analyzed.The levels of NGAL,UACR and eGFR in patients with different renal injury grades were compared.Spearman correlation was used to analyze the relationship between NGAL level and UACR and eGFR.Results The levels of NGAL and UACR in DN group were higher than those in DM group,and the level of eGFR was lower than that in DM group(P<0.05).ROC curve analysis showed that NGAL detection had a definite diagnostic value for early DN disease.The AUC value was 0.836,the sensitivity was 90.14％,the specificity was 87.62％,the Youden index was 0.875,and the 95％CI interval was 0.682-0.914.The levels of NGAL,UACR and eGFR in patients with different renal injury grades were statistically significant(P<0.05).The NGAL and UACR indexes from high to low were stage Ⅲ>stage Ⅱ>stage Ⅰ,and the eGFR indexes from high to low were stage Ⅰ>stage Ⅱ>stage Ⅲ.Spearman correlation analysis showed that NGAL level in DN patients was positively correlated with UACR(r=0.502,P=0.000)and negatively correlated with eGFR(r=-0.473,P=0.000).Conclusion NGAL index has good diagnostic value for early DN disease,and its parameter level is closely related to the degree of renal injury in patients.关键词
