A correlation study between the degree of vestibular and cochlear endolymphatic hydrops and clinical stage in patients with Meniere's disease

Abstract
Objective:To investigate the correlation between the degree of vestibular and cochlear endolymphatic hydrops and the clinical staging of Meniere's disease patients.Methods:A total of 172 ears of 161 patients with Meniere's disease were selected,and all of them were given a double dose of gadolinium and underwent a delayed MRI scan 4 h later and perform a pure-tone auditory threshold test(PTA).Clinical staging was performed based on the PTA results of the affected ears according to the criteria of the Chinese guidelines for the diagnosis and treatment of Meniere's disease.Mean hearing thresh-old≤25 dBHL is clinical stage Ⅰ,mean hearing threshold>25~40 dBHL is clinical stage Ⅱ,mean hearing threshold>40~70 dBHL is clinical stage Ⅲ,mean hearing threshold>70 dBHL is clinical stage Ⅳ.The assessment of the degree of en-dolymphatic hydrops was based on the visual assessment method proposed by Bernaerts classifying cochlear hydrops into 3 grades and vestibular hydrops into 4 grades.Then the correlation between the degree of vestibular and cochlear endolymphatic hydrops and the clinical staging was analyzed.Results:There were significant differences in the degree of hearing loss due to the site of vestibular and cochlear endolymphatic hydrops in 161 patients(172 ears in total),and there was a positive correla-tion between the different degree of vestibular and cochlear endolymphatic hydrops and the clinical staging of Meniere's dis-ease patients(P<0.05).Conclusion:There was a correlation between the site and degree of vestibular and cochlear endolym-phatic hydrops and the clinical stage of Meniere's disease patients,and changes in the site and degree of vestibular and cochlear endolymphatic hydrops assessed by gadolinium-enhanced inner-ear MRI may help patients with clinical staging.关键词
