多参数MRI联合临床指标预测ⅢC1p期宫颈癌预后的价值
The value of multi-parameter MRI combined with clinical prognostic parameters in predicting the prognosis of stage ⅢC1p cervical cancer

Abstract
Objective:To investigate the prognostic value of multi-parameter MRI combined with clinical prognostic parameters in prognosis of 2018 FIGO stage ⅢC1p cervical cancer.Methods:A total of 71 patients with cervical cancer(2018 FIGO stage ⅢC1p)were enrolled from Dec.2014 to Jul.2018.All patients underwent radical hysterectomy(RH)+lymph node dissection+adjuvant therapy.The preoperative clinical indicators and postoperative pathological indicators of the patients were collected,and the apparent diffusion coefficient(ADC)(ADCmean,ADCmin,ADCmax)and dynamic contrast-enhanced(DCE)MRI quantitative parameters(Ktrans,Kep,Ve)of the primary tumor were measured and recorded.Using Kaplan-Meier to plot survival curves and Cox proportional risk model to determine independent influencing factors.Results:As the follow-up date,15 patients developed progression(2 cases of local recurrence,8 cases of distant metastasis and 5 cases of death).The median progression-free survival(PFS)was 42 months(6～68 months).The 1-year and 3-year PFS rates were 94.4%and 78.6%,respectively.Through analysis of independent influencing factors(LNR and ADCmin)predicting PFS for cervical cancer patients in ⅢC1p stage,LNR>10%and ADCmin<0.61×10-3 mm2∕s correlated with worse PFS.Conclusion:The ADCmin of primary tumor and LNR are independent influencing factors of PFS in stage ⅢC1p cervical cancer.Patients with ADCmin<0.61×10-3 mm2∕s and LNR>10%have a poor prognosis.关键词
