Abstract

Objective To investigate the influencing factors of enteral nutritional intolerance in patients with gastric cancer after surgery.Methods Ninety-nine patients with gastric cancer undergoing enteral nutrition therapy were enrolled.Nutrient so-lutions(glutamine,Peptisorb,Kangquali,and Ruixian)were used according to the patient's postoperative recovery of gastroin-testinal function.Among them,saline(0.9%concentration)was used before enteral nutrition in 54 cases.According to whether patients had symptoms of intolerance after surgery,they were divided into the tolerance group and the intolerance group.The symptoms of the intolerance group(nausea and vomiting,abdominal pain,bloating,diarrhea),the first time to get out of bed af-ter surgery and the time to first use enteral nutrition,as well as the changes in biochemical indexes(total protein,albumin)be-fore and after the use of enteral nutrition were observed.Results Intolerance was developed in 40 patients,including 32 cases of abdominal distention,22 cases of abdominal pain,8 cases of diarrhea,and 8 cases of nausea and vomiting.There were 59 cases without intolerance symptoms.Intolerance was developed in 25 patients with Peptisorb,14 with glutamine,and 2 with Kangquan-li.There were significant differences in intraoperative blood loss,postoperative first time out of bed and actual enteral nutrition dosage in the enteral nutrition-tolerant group(P<0.05).Absolute lymphocyte values and total protein were elevated in all pa-tients(P<0.05).The albumin in the tolerant group was elevated and the difference was statistically significant(P<0.05).The results of multivariate logistic analysis showed that intraoperative bleeding(OR=1.018)was an independent risk factor for enteral nutritional intolerance in patients with gastric cancer.Conclusion High actual infusion of enteral nutrition and intraoper-ative bleeding in gastric cancer patients are risk factors for intolerance of EN,which will delay postoperative recovery.
