重症肺炎机械通气患者气道分级管理结合早期分阶段计划性康复锻炼的应用效果
Application effect of airway grading management combined with early staged planned rehabilitation exercise in patients with severe pneumonia mechanical ventilation

Abstract
Objective:To explore the application effect of airway grading management combined with early staged planned reha-bilitation exercise in patients with severe pneumonia mechanical ventilation.Methods:110 patients with severe pneumonia undergoing mechanical ventilation were selected as the research subjects,and they were divided into observation group and control group according to the different nursing methods,with 55 cases in each group.Both groups were given routine nursing intervention.On this basis,the control group was given airway grading management combined with routine rehabilitation training.The observation group was given air-way grading management combined with early staged planned rehabilitation exercise,the intervention time was 14 days.The mechanical ventilation,pulmonary infection control,ICU stay time,hospitalization,disease improvement status[acute physiology and chronic health status score Ⅱ(APACHE Ⅱ)and clinical pulmonary infection score(CPIS)]],lung function[forced expiratory volume in 1 second(FEV1),FEV1/forced vital capacity(FVC),peak expiratory flow(PEF)],grip strength and muscle strength[Medical Research Council Muscle Strength Rating(MRC Score)],and incidence of adverse events were compared between the two groups.Results:The mechanical ventilation time,pulmonary infection control time,ICU stay time and hospitalization time in the observation group were shor-ter than those in the control group(P<0.05).The scores of APACHE Ⅱ and CPIS in the observation group were lower than those in the control group after 7 and 14 d of intervention(P<0.05).The levels of PEF,FEV1,FVC,grip strength and MRC muscle strength grading scores in the observation group were higher than those in the control group(P<0.05).The incidence of adverse events in the observation group was lower than that in the control group(P<0.05).Conclusion:In patients with severe pneumonia undergoing me-chanical ventilation,airway grading management combined with early staged planned rehabilitation exercise can improve lung function,promote the recovery of grip strength and muscle strength,accelerate the rehabilitation process,and reduce the risk of adverse events.关键词
