低容量胰腺中心腹腔镜胰十二指肠切除术42例治疗体会
Surgical experience of laparoscopic pancreaticoduodenectomy in a low-volume pancreatic center with 42 cases

Abstract
Objective To discuss and summarize the surgical experience of laparoscopic pancreaticoduodenectomy(LPD)in a low-volume pancreatic center.Methods A retrospective analysis was performed on the clinical data of 42 patients who underwent LPD from May 2020 to May 2023 in Affiliated Suzhou Hospital of Nanjing Medical University.Among them,23 were male and 19 were female,with average age(65.4±9.6)years.Results LPD were performed successfully in 36 cases;the other 6 cases were converted to open pancreaticoduodenectomy.The average operation time was(302.3±41.6)min,and the pancreaticojejunostomy time was(16.3±3.0)min.The median blood loss was 210(200,340)mL.The average postoperative hospitalization time was(21.1±4.2)d.Postoperative complications included 15 cases of biochemical leakage(no special treatment required),4 cases of grade B pancreatic fistula and 1 case of grade C pancreatic fistula.Pathological results showed that there were 22 cases of pancreatic ductal adenocarcinoma,5 cases of cholangiocarcinoma,7 cases of adenocarcinoma perampullaire and 8 cases of duodenal adenocarcinoma.Conclusion LPD is a complex surgical procedure with a relatively longer learning curve.Sufficient experience in laparoscopic surgery is necessary before conducting LPD.Certain optimization of pancreaticojejunostomy,with application of the omental pad,these measures can gradually shorten the operation time,improve surgical safety and smoothly navigate the learning curve,enabling low-volume pancreatic centers to smoothly carry out LPD.关键词
