Evaluation value of DTI for cognitive dysfunction in Parkinson's disease

目的 探讨磁共振扩散张量成像(DTI)技术对帕金森病(PD)认知功能障碍的评价作用,分析PD相关认知障碍的影响因素.方法 选取2021-01-2022-12南京医科大学第一附属医院诊治的PD患者120例,采用蒙特利尔认知评估量表(MoCA)评估患者的认知功能,并根据患者有无认知功能障碍分为认知障碍组67例和认知正常组53例,均接受DTI检查,比较2组各向异性分数(FA)值和平均扩散率(MD),并分析认知功能障碍的危险因素.结果 2组苍白球、脑桥的FA值比较差异均无统计学意义(P>0.05),认知障碍组尾状核头、黑质、丘脑的FA值与认知正常组比较明显较低(P<0.05).2组苍白球、尾状核头、脑桥、黑质、丘脑的MD值比较,差异均无统计学意义(P>0.05).认知障碍组受教育年限明显短于认知正常组(P<0.05),认知障碍组HAMD评分、HAMA评分、UPDRS-Ⅲ评分明显高于认知正常组高(P<0.05).经二元Logistic回归分析发现高HAMA评分、高UPDRS-Ⅲ评分是认知障碍的危险因素(P<0.05),受教育年限长是认知障碍的保护因素(P<0.05).结论 DTI技术的尾状核头、黑质、丘脑的FA可评估PD患者的认知功能障碍,PD患者认知功能障碍与焦虑、抑郁、受教育年限有关.

Objective To investigate the evaluation value of magnetic resonance diffusion tensor imaging(DTI)technology on cognitive dysfunction in Parkinson's disease(PD),and to analyze the influencing factors for cognitive impairment related to PD.Methods In the 120 PD patients diagnosed in the First Affiliated Hospital of Nanjing Medical University from January 2021 to December 2022,the Montreal cognitive assessment scale(MoCA)was used to evaluate the cognitive function,and were divided into cognitive dysfunction group(67 cases)and normal cognitive function group(53 cases),which were examined by DTI.The fractional anisotropy(FA)and mean diffusivity(MD)were compared,and the risk factors for cognitive dysfunction were analyzed.Results The FA values of the pallidum and pontons were not statistically significant(P>0.05),and the FA values of the caudate head,substantia nigra and thalamus in the cognitive dysfunction group were significantly lower than those of the normal cognitive function group(P<0.05).Comparison of MD values of globus pallidum,caudate head,pons,substantia nigra,and thalamus was not significant(P>0.05).The years of education in the cognitive dysfunction group were significantly shorter than those in the normal cognitive function group(P<0.05),and the HAMD score,HAMA score,and UPDRS-Ⅲ score were significantly higher in the cognitive dysfunction group than those in the normal cognitive function group(P<0.05).After binary Logistic regression analysis,high HAMA score and high UPDRS-Ⅲ score were risk factors for cognitive impairment(P<0.05),and long years of education was a protective factor for cognitive impairment(P<0.05).Conclusion The FA of caudate nucleus,substantia nigra and thalamus of DTI technology can evaluate the cognitive dysfunction of PD patients,which is related to anxiety,depression and years of education.
