目的 探讨超声速度向量成像(VVI)技术评估缺血性脑卒中患者颈动脉弹性的价值.方法 纳入2020-05-2023-01联勤保障部队第九六四医院的92例缺血性脑卒中患者为观察组,选取同时期健康体检者50例为对照组,比较2组一般临床资料、常规超声参数[颈动脉内膜中层厚度(IMT)、颈动脉直径(CAD)、颈动脉血流速度(CAV)、颈动脉血流量(CAF)]、VVI参数[颈动脉纵向应变(LS)、纵向应变率(LSR)、径向应变(RS)、径向应变率(RSR)],采用Pearson相关性系数分析观察组患者VVI参数与血压、血脂指标及常规超声参数的相关性.结果 观察组收缩压、舒张压、总胆固醇、低密度脂蛋白(LDL)、甘油三酯水平均高于对照组(P<0.05),高密度脂蛋白(HDL)水平低于对照组(P<0.05).观察组IMT、CAD分别为(1.23±0.25)mm、(6.53±0.72)mm,大于对照组的(0.79±0.15)mm、(6.01±0.65)mm(P<0.05),CAV为(29.72±4.85)cm/s,小于对照组的(33.46±5.28)cm/s(P<0.05).观察组颈动脉LS、LSR绝对值及RS、RSR值分别为(18.23±2.35)%、(1.32±0.21)s-1、(23.45±4.12)%、(1.87±0.34)s-1,小于对照组的(22.76±2.84)%、(1.57±0.26)s-1、(28.78±4.63)%、(2.23±0.38)s-1(P<0.05).Pearson相关性分析显示,观察组患者LS、LSR绝对值及RS、RSR值与收缩压、总胆固醇、甘油三酯、IMT、CAD均呈负相关(P<0.05),与CAV均呈正相关(P<0.05).结论 VVI技术可定量评估缺血性脑卒中患者颈动脉弹性,且其定量参数与血脂、血压指标及颈动脉常规超声参数有一定相关性.

Objective To investigate the value of ultrasound velocity vector imaging(VVI)on evaluating carotid artery elasticity in patients with ischemic stroke.Methods Totally 92 patients with ischemic stroke in the 964th Hospital of the Joint Logistic Support Force from May 2020 to January 2023 were included in observation group,and 50 healthy subjects with physical examination during the same period were selected as control group.General clinical data,conventional ultrasound parameters(carotid intima-media thickness(IMT),carotid artery diameter(CAD),carotid artery velocity(CAV),carotid artery flow(CAF)),VVI parameters(carotid longitudinal strain(LS),longitudinal strain rate(LSR),radial strain(RS),radial strain rate(RSR))were compared between the two groups.Pearson correlation coefficient analysis was used to analyze the correlation between VVI parameters and blood pressure indicators,blood lipids and conventional ultrasound parameters in observation group.Results Systolic blood pressure,diastolic blood pressure,total cholesterol,low density lipoprotein(LDL)and triglyceride in observation group were higher than those in control group(P<0.05),while high density lipoprotein(HDL)was lower than that in control group(P<0.05).The IMT and CAD in observation group were(1.23±0.25)mm and(6.53±0.72)mm,respectively,both greater than(0.79±0.15)mm and(6.01±0.65)mm in the control group(P<0.05),while the CAV was(29.72±4.85)cm/s,which was less than(33.46±5.28)cm/s in the control group(P<0.05).The absolute values of carotid LS,LSR,RS and RSR in observation group were(18.23±2.35)%,(1.32±0.21)s-1,(23.45±4.12)%,and(1.87±0.34)s-1,respectively,all less than(22.76±2.84)%,(1.57±0.26)s-1,(28.78±4.63)%,and(2.23±0.38)s-1 in the control group(P<0.05).Pearson correlation analysis showed that the absolute values of LS,LSR,RS and RSR in observation group were negatively correlated with systolic blood pressure,total cholesterol,triglyceride,IMT and CAD(P<0.05),and were positively correlated with CAV(P<0.05).Conclusion VVI can quantitatively evaluate the carotid artery elasticity in patients with ischemic stroke,and these quantitative parameters have a certain correlation with blood lipids,blood pressure indicators and conventional carotid ultrasound parameters.
