Effect of modified encephalo-duro-arterio-synangiosis surgery on cerebral hemodynamic and nerve functions in patients with internal carotid artery or middle cerebral artery stenosis

Abstract
Objective To discuss the effect of modified encephalo-duro-arterio-synangiosis(EDAS)surgery on cognitive and nerve function in patients with internal carotid artery or middle cerebral artery occlusion.Methods 54 patients with internal carotid artery occlusion or middle cerebral artery occlusion admitted to the Second Affiliated Hospital of Xinjiang Medical University from January 2021 to April 2022 were analyzed retrospectively.According to the choices of patients and their families,patients with the same surgical indications were divided into surgery group and drug treatment group according to different treatment methods.The drug treatment group was the control group,and the changes in cerebral hemodynamics,cognitive function,and neurological function of the two groups of patients were observed 6 months after surgery.Results Compared with baseline,after 1 year of follow-up,the cerebral blood flow(CBF)of patients in the surgical group significantly increased,with a statistically significant difference(P=0.006).The mean transit time(MTT)and peak time(TTP)of patients in the surgical group significantly decreased,with a statistically significant difference(P=0.000 1,P=0.002).Compared with baseline,the Montreal Cognitive Assessment Scale(MoCA)and Mini Mental State Scale(MMSE)scores of patients in the surgical group improved significantly after 1 year of follow-up(P＜0.001).Compared with baseline,the mRS scores of patients in the surgical group decreased significantly after 1 year of follow-up(P=0.002).Conclusions The improved EDAS surgery group have a better prognosis,with simple procedures and fewer perioperative complications.The modified EDAS is a safe and effective treatment for patients with intracranial atherosclerotic occlusion.关键词
